• According to Canon Law, to be a Godparent, an individual must be a baptized and confirmed Catholic, over the age of 16, who is a registered member of a parish.

• If the perspective Godparent is not a member of the Catholic Community of St. Jude, we will need a verification letter from the parish where they are currently registered.  This form must be filled out by their parish and returned no later than the Wednesday prior to the scheduled baptism.  The verification form can be found HERE.

• Only one Catholic Godparent is required, but if two are selected, one must be male and the other must be female.

• Provided one Catholic Godparent is selected, a non-Catholic may serve as a Christian Witness.  See below.

Provided that one Catholic Godparent is selected and meets the requirements, the other representative for the child can be what is called a "Christian Witness'.

A Christian Witness is a non-Catholic who believes in Christ, was baptized as a Christian, and remains active in their faith.  They must be over the age of 16.

We ask that THIS VERIFICATION FORM be filled out and submitted to St. Jude no later than the Wednesday prior to the scheduled baptism.
